Pier40Baseball Day Camp is licensed by the City of New York Department of Health and Mental Hygiene.
We are required to have current health forms for all campers on file. Last year's form is not acceptable
and campers cannot attend until we have the form.
Please provide us with a copy of any form that you submitted for school or
another camp within the past year. If you do not have a copy of this form, please ask your doctor's
office to provide their standard form.
