The Health Deptarment requires that we have current health forms on file for all campers.
Last year's forms are not acceptable and campers cannot attend unless we have the forms.
There are two pages. For your convenience, if you have a copy of a similar form signed by your
child's doctor, you can use that one in place of the page that requires a doctor's signature.
